Dear Mentee,

We are pleased that the Mentoring Program has been part of the Plan for Success Program. The intent of the Mentoring Program was to pair you with an individual who has expert knowledge or skills that could boost your knowledge and prepare you for new experiences. The program is confidential in nature and is managed entirely by you.

With that said, we would like to gain general knowledge of how the program is working, what benefits you may have experienced, and perhaps some ideas that can be shared with the Leadership Team about the program.

We’d like to ask you a few questions about your experience with the mentoring program to date. Your answers will be kept confidential. We will consolidate all answers into one general ‘snapshot’ on how the program is benefiting the participants of the Plan for Success program.
